Risk Management and Insurance

The office of Risk Management has the responsibility of identifying, evaluating and managing any loss exposures faced by the University.

This can range from a fire in one of the Living Learning Centers to a computer stolen out of one of the computer labs. We provide all administrative services related to University insurance, which includes the following:

Medical/Maternity Leave

Medical Leave

In order to streamline procedures and assist employees with medical leaves, all employees (full-time, part-time, clerical, administrative, faculty) requesting a medical leave should submit a Medical Leave Request Form to Genny Hughes, Disability Claims Manager. Employees should give their supervisor at least 30 days advance notice of the need for a medical leave. If the 30-day notice is not possible, notice should be provided as soon as possible.

Maternity Leave

Maternity Leave is treated as any other Medical Leave(refer to TAP No.15). The standard medically necessary time off is 6 or 8 weeks, depending on type of delivery. Sick leave and disability benefits are based on your employment classification. Employees covered by a collective bargaining agreement should refer to their contract.

Insurance Information

Automobile Coverage

This insurance covers any damages or losses to University owned, leased or rented vehicles. Any employee involved in an auto accident or fender bender should notify Genny Hughes at 412-396-6677.

When employees are renting vehicles they should:

  1. Use a University credit card when possible and
  2. Waive the rental company's insurance, because they are covered by the University insurance.

The exception to this is if you are renting a vehicle in Canada or a foreign country, then you should purchase the rental service insurance.

Property and General Liability

This insurance is designed to protect the University and employees from a wide variety of liability exposures.

Liability insurance sometimes is called third-party insurance because the insurance protects the insured (Duquesne) against suit by a third party.

Professional Liability

This insurance indemnifies the University for any loss sustained because of an error or oversight on the part of an employee in the course of his/her performing University activities. This includes all students during University sponsored internships.

Student Health Insurance

UNIVERSITY POLICY REQUIRES ACTION BY ALL FULL-TIME STUDENTS REGARDING HEALTH INSURANCE COVERAGE

In light of the rising cost of health care and the negative effects of inadequate health care on a student’s academic experience, the University instituted a policy regarding student health insurance. Effective August 1, 2006, all full time students must have medical insurance coverage and provide the University with verifiable proof of coverage. If a student does not provide proof or enroll in one of the two plans offered by the University, they will be enrolled in Plan 2. Their student account will be billed for the cost of the insurance.

You must provide proof of coverage or enroll in one of the two plans one of the following ways:

1. Go to: www.hulseqm.com/duquesne. If you have not received a letter in the mail with this information you please call 1-800-791-9420 or 412-396-6677.

2. Print the waiver/enrollment form, complete it and follow mailing directions on the form.

3. Call 1-800-791-9420 and give your insurance inforamtion.

Vehicle Use

Any employee or student wishing to drive a University vehicle must complete an Application for Driving Privileges.

Worker's Compensation

Program Guidelines

Duquesne University has a responsibility, as defined by the Pennsylvania Workers' Compensation Act, to provide wage loss compensation and payment for reasonable medical services and supplies for faculty, staff and student employees who sustain work-related injuries or illnesses during the course of employment.

To meet this obligation, Duquesne has an established program that evaluates such incidents to determine fiscal liability, proper medical management and possible solutions to prevent future injuries or accidents. In compliance with the provisions of the Act, the University has elected to be a self-insured employer.With this election, the University retains a Third Party Administrator to assist in the administration of this program. Duquesne has implemented procedures for reporting and obtaining medical treatment for work-related injuries and illnesses.

These procedures include the utilization of a designated Panel of Medical Providers to treat such incidents. Compliance with these procedures will provide effective medical treatment while efficiently minimizing the possible loss of work time.

It is the responsibility of the Office of Risk Management to work with the affected employees, their supervisors, General Counsel and the Third Party Administrator to resolve claims and return employees to productive University employment.

Reporting Injuries & Obtaining Medical Care

Procedures for reporting work related injuiries and obtaining medical care:

  1. Injured/ill employee should report occurrence to supervisor immediately (but at least within 24 hours). An Accident Investigation Report must also be completed. The Supervisor must review the report, sign it, and promptly send it to the Risk Manager, Fisher Hall. If the employee needed immediate medical care and was put off of work prior to completing this form, one will be faxed or mailed to the employee as soon as they are able to complete it. An Accident Investigation Report must be completed even if the employee declines medical care.
  2. Supervisor should assess immediacy of medical care. If injury is minor (which includes mild abrasions, small cuts, rashes, minor strains), send the employee to Duquesne's Health Services for initial evaluation during available hours listed below. If more severe, contact the campus Emergency Number at 412-396-2677 (X2677) for assistance in arranging transport to Mercy Hospital.

    Summer: Monday thru Friday, 8:30 a.m. to 4:30 p.m. Fall, Winter, Spring: Monday thru Thursday, 7:30am to 7:00 p.m.; Friday, 7:30 a.m. to 4:30 p.m.

  3. Supervisor should contact the Office of Risk Management at 412-396-6677 to report all injuries/illnesses as soon as possible, stating employee name, what happened, and if employee sought medical treatment. Leave a voice or e-mail message hughes@duq.edu, if no one is available.
  4. The Office of Risk Management will contact the employee to determine status of injury and necessary medical follow-up. A letter may be sent to the employee explaining pertinent workers' compensation issues and the medical providers that must be used according to Duquesne's Medical Provider Notice. She will also interact with physicians, supervisor, and employee to determine work status, physical restrictions, if applicable, and any temporary, light-duty positions needed to accommodate such restrictions. She will continue this process for the duration of the injury or illness.
  5. The Office of Risk Management will investigate causes of injuries or illnesses, and will review and enforce corrective actions to eliminate future incidents.
  6. The Supervisor should direct all medical provider reports or bills to the Office of Risk Management. Any unusual circumstances should also be reported for investigation.
  7. The Supervisor should make periodic inquiries to the employee to check physical status and offer support.

Please contact the Office of Risk Management at 412-396-6677 if the employee has any physical problems or illnesses which may affect their ability to perform their job, even if these problems were not caused by work.

Duquesne University Medical Provider Notice for Work-Related Injuries/Illnesses
Effective: July 1, 2008

If you experience a work-related injury or illness, your employer, Duquesne University, through its Third Party Administrator, CompServices, Inc., P.O. Box 3460, Pittsburgh, PA 15230, (412) 402-4248, shall provide payment for reasonable surgical and medical services, services rendered by physicians or other health care providers, medicines and supplies, and orthopedic appliances and prostheses, as and when needed.

These services will be provided to you under Duquesne University's Workers' Compensation program as described below:

To insure that your medical treatment will be paid by Duquesne University, you are required to visit one of the one of the providers on our Panel Of Physicians.
